This registration form is for Volunteers that will have contact with players, including Board Members/Coordinators, Coaches, Team Managers and Locker Room Monitors.
Before completing this registration process:
1. Register with USA Hockey for the current season and enter your own USA Hockey confirmation number when prompted. Coaches should follow the Ice Players and Coaches process while Board Members, Team Managers and Locker Room Monitors may register as an Ice Manager/Volunteer. If you are both coaching and volunteering please register with USA Hockey as a Coach.
2. Make sure your SafeSport training is up to date and have your SafeSport training completion certificate on hand. SafeSport training is required every year.
3. Complete your MN Hockey background check and be prepared to enter the date completed. Background checks are required every two years.
4. Coaches will also need to complete Concussion Training and fill out a Concussion Training Certification form. The certification form is available on the AHYHA coaches page. Training is required every two years.
